(married parents or they have joint custody by virtue of court order or they have joint custody by virtue of a joint declaration to the Court)

Traveling with both parents

• “marriage book” of the traveling parents in which the minor’s name appears

• an extract from the Parental Authority Register (not older than 6 months)

Traveling with one parent only

• “marriage book”⁵ or long extract of the minor (not older than 6 months)

• fully completed consent form and signed by the legal representative(s)

• copy of the identity document or passport of the parent who is not traveling.

Traveling with another person or institution/organization

• long extract of the minor (not older than 6 months) or “marriage book” ⁶

• fully completed consent form and signed by the legal representative(s)

• copy of passport or identity document of both parents

• identity document of the accompanying adult
