Child is acknowledged by father, but only mother has custody; or it has been determined by a Court Order, that only the mother or father is charged with parental authority.

Traveling with the parent with parental authority

• Long extract of the minor (not older than 6 months)

• Extract from the Parental Authority Register (not older than 6 months)

Minor traveling alone or with someone other than the parent in with parental authority:

• Long extract of the minor (not older than 6 months)

• Extract from the Parental Authority Register (not older than 6 months)

• Completed consent form 

• Copy of a valid identity document or passport of the competent parent

• Identity document of the accompanying adult
